If you currently accept VISA/MasterCard, select YES to item (A) and be sure to include your 3 most recent months of VISA/MasterCard statements with your signed application.

For AMEX and Discover, we will, upon your request, apply for the American Express and/or Discover services, at no extra charge (from us). Discover will bill their customers $25.00 directly as a "One Time Application Fee".  This is Discover's charge not ours, but we want you to be aware of it.   In addition, if you ask for AMEX, we will email you the one-page AMEX application form.  Please FAX this in with your completed application (Discover does not need a separate form).  AMEX will charge a $5.00 MINIMUM (If you do not accumulate $5.00 in fees for the month, they charge your $5.00 anyway).  AMEX and Discover give EACH merchant a UNIQUE rate, based on business type and credit history.  Please note that the acceptance by these companies is not automatic, and that the percentage rate they charge is dependent on your credit history.
